New Patients

Welcome to Pohala! For your first appointment, you may schedule at (503) 572-4196. We do our best to answer the phone immediately, but if we do not please leave a message and we'll get back to ASAP. It is helpful if you state days and times of your availability.

On the day of your first appointment either have all our paperwork ready or arrive 15-20 minutes early to fill it out. The New Patient forms are available as Acrobat PDF files at right and you can print and complete them before your visit.

At time of visit please have insurance card and information available. Remember to check on your benefits/coverage/copays/deductibles beforehand. Ultimately you are responsible to know. Your insurance company will be the last say on billing so develop a working relationship here. We are available for questions and guidance.

If paying at time of service we request at least a $100 deposit.

You will be charged $75 for no-show fees or rescheduling less than 24hours before time of appointment.
